Before your first visit, review the below information to be prepared for your appointment:

Please bring your medical insurance cards and driver’s license or photo ID to every appointment.
We will collect all co-payments, co-insurance, and deductibles at your appointment.
Bring a current list of all medications, including eye drops.
Keep a copy of all lab results, notes, and papers given to you by your doctor.
We will be dilating BOTH your eyes. You may need to bring a driver, as the dilation could cause vision problems.
Allow at least 1 – 2 hours for your appointment. The initial visit might take longer.
We will notify your referring doctor of our findings.

Private payment patients, please call our main office at 225-768-8833 to discuss any self-pay fees before your appointment.
